The Optum Care supportive care difference

Optum Care supportive care programs offer you and your family specialized medical care. We focus on caring for you as a whole person to help with your pain, stress, tiredness or symptoms. The goal is to help improve quality of life for you and your family.

We help you better understand your health

We offer care when and where you need it

We offer care in your home, in the hospital or in a skilled nursing facility. 

Supportive care

Our team will help you every step of the way. No matter how complex your care, we work to simplify your path to better health.

From supportive care, you can expect:

  • A focus on helping you have the best possible quality of life
  • Specialized medical care for people living with long-term medical problems or an illness
  • Care coordinators and case managers ready to help with your referrals, appointments and reminders
  • Help finding care for your symptoms such as weakness, tiredness or pain
  • Support for your family and caregivers

Optum Care programs offer added support to you at no extra cost. Our programs include:

  • Optum Care health management services add to the care that your provider gives you. We may offer extra services and support after a move from a hospital or nursing facility to home. 
  • Optum Care case management is for members who have complex or long-lasting medical problems. We help find the specialized medical care and support you need. Better yet, we may bring these services to you in the comfort of your own home. 
  • Optum Care also offers disease and case management services for members with certain health conditions. We help coordinate the care you need to help you reach your health goals.

The care team will spend time with you to better understand:

  • What are your health care goals and wishes?
  • What activities do you enjoy and would like to keep doing?
  • What is most important for you in life as you think about your health care choices?

If you or a loved one are living with a long-term medical problem or an illness, Optum Care supportive care programs may help. You may want to talk to your provider about starting supportive care.

Your rights and roles

The Optum Care case management program pledges to help you get care and information so that you can take charge of your health.

You have the right to:

  • Know about our program, services and all we have to offer.
  • Work with your provider to make health care choices and have our support.
  • Know about treatment options and talk about those choices with your provider.
  • Get information that’s easy to understand. If you don’t speak English, or if you have physical or mental health concerns or concerns about your mental abilities, we’ll provide support so that you can make informed health care decisions.
  • Have your personal information kept private and know how we protect your privacy.
  • Know who can see your information.
  • Be treated with courtesy and respect by our staff.
  • Know which staff members are managing and monitoring your illness, and how you can make a change.
  • Choose not to take part in our program and in any part of your care plan. You may quit the program at any time.
  • Know about any changes in our program services or termination of our services.
  • Know how to file complaints, how we answer questions and work out issues as well as how long it will take for us to take care of your issue, if you have one.

Know what to do if you feel your rights have been violated, or you have been treated improperly.

We’re here to support the relationship and treatment plan you have set up with your provider.

As part of your commitment to our program and services — and to your health — your role is to:

  • Follow the plan you made with your case manager. Tell your case manager if you can’t follow your plan or if you would like to change it.
  • Follow your provider’s directions about your illness. Talk with your provider before changing your treatment plan.
  • Give us the information we need to provide you with the tools and services you might need.
  • Let your provider and Optum Care know if you choose to quit our program.
  • Tell us when you're in the hospital, when your medications change or when you get a new provider.

Call your provider when needed. Call 911 if you have an emergency. Your Optum Care registered nurse case manager will talk to you about your choices and ask for your input in your case management plan.

The supportive care team works hand-in-hand with your current care team. We work with your providers to help you take care of your pain and symptoms. We also make sure your supportive care fits into your entire care plan.
