Optum Arizona

Login or sign up for our new MyChart patient portal

* MyChart can be used to message your provider, request refills, request or cancel upcoming appointments, view your medical records, pay bills, and more.

* This new patient portal is for all visits on or after October 1 (excluding Apricus, Optum Primary Care Baseline, Dobson and San Tan locations). 

* For Apricus, Optum Primary Care Baseline, Dobson and San Tan locations, this patient portal is for all visits on or after December 1, 2024.

 

For bill pay and patient portal access prior to moving to MyChart:

* If you receive care from Optum Arizona (excluding the below locations) and your visit was before Oct. 1, 2024, please call 1-800-765-8959, TTY 711 to pay a bill.

* If you receive care from Apricus, Optum Primary Care Baseline, Dobson and San Tan locations and your visit was before December 1, 2024, please call 1-800-765-8959, TTY 711 to pay a bill.

 

* If you receive care from Optum Arizona (excluding the below locations) and your visit was before Oct. 1, 2024, view historical medical records here. 

* If you receive care from Apricus, Optum Primary Care Baseline, Dobson and San Tan locations and your visit was before December 1, 2024, view historical medical records here.

    Unfortunately, due to system issues, online bill pay is unavailable and patient statements are currently on hold. If you have billing questions, please call 1-800-765-8959, TTY 711.

     

    If your visit was before Oct. 1, 2024 and your PCP was Dr. Vogeler or John Moore from Sandy Family Practice before moving in to Optum, visit Healow to view prior medical records or pay a bill.
